Freshmen Admission Procedure
Click the "Proceed" button below and login to your account.
To confirm that you are availing of the slot, tick the “I Confirm” check box, then click the "Update Record" button. Update the needed information. Once done, you will be given an opportunity to edit the incorrect data that were inadvertently submitted. You may print your Student’s Personal and Academic Records (SPAR) before the registration day assigned to you.
Proceed to PLM on the registration date indicated in your account (pls. stand by for it) and submit the following documents:
PSA-issued Birth Certificate (Original)
SHS F-138 (Report Card): If the original (with 4th quarter grades) is not yet available, submit the school-certified copy (with 3rd quarter grades).
Parent's/Legal guardian’s Voter's Registration with latest voting record (with thumbmark) issued by the COMELEC; and Brgy. Certificate of Residency, with a Sketch of Address duly authenticated by the Brgy. Chair.

N.B.: Attached to the Enrollment Procedure Form, which the PLM Registrar will give you, is a request slip for the SHS F-137 (Transcript of Records).
Following the steps in the Enrollment Procedure Form, proceed with your enrollment. This will remain provisional until you complete all the requirements, including the Physical Exam below.
Upon confirmation of your slot under step no. 1, you need to undertake the lab exams for Chest x-ray (PA view) and CBC. Download the Medical Request Form here.
Manilans may undertake their lab exams at the Manila-operated hospitals for free (Pls. check the availability of the PLM-Ospital ng Maynila, Ospital ng Tondo, Ospital ng Sampaloc, Ospital ng Sta. Ana, and Gat Andres Bonifacio Medical Center).
Non-Manilans may undertake their lab exams in any DOH-accredited diagnostic laboratories.
On a first-come, first-serve basis, the University Clinic will conduct your Physical Exam. Pls. bring the lab results.
